Summary: This bill would require the Department of Social Services (DSS) to create web forms for any forms that must be completed by an applicant to apply for benefits and services through DSS. The web forms would allow applicants to directly input information and electronically sign applications and forms online. DSS would be required to continue to accept paper applications and forms that mailed, faxed, or delivered in person.

A Local Law to amend the administrative code of the city of New York, in relation to requiring the department of social services to create web forms for all application forms

 

Be it enacted by the Council as follows:

 

Section 1. Chapter 1 of title 21 of the administrative code of the city of New York is amended by adding a new section 21-153 to read as follows:

§ 21-153 Benefits and services applications web forms. a. Definitions. For purposes of this section, the term “application form” means any form that must be completed by an applicant to apply for any benefit or service through the department.

b. The department shall create and maintain web forms for all application forms, unless doing so would violate any applicable provision of state or federal law. Such web forms shall contain interactive form fields allowing applicants to directly input information and electronically sign and submit such forms. The department shall also continue to accept forms that are mailed, faxed, or delivered in person.

§ 2. This local law takes effect 120 days after it becomes law.
